Initial Telephone Interview
Counseling and Psychotherapy begins with an initial 15 to 20 minute telephone interview
to help me understand the nature of your concerns so I can make an informed decision about the best treatment options for you or your family’s needs. After our brief telephone interview, we can schedule an appointment or I can provide you with an appropriate referral. This initial telephone interview is confidential and free of charge.
Scheduling Your Appointment
All services are by appointment only. I appreciate your being on time for your appointments so I can give you all the time you need for therapy.
You can schedule appointments by calling the office at 970-761-0048. If you do not reach me in person, please leave a message on my confidential voice mail and I will usually return calls within 24 hours accept on weekends and holidays.
Online scheduling is also available for existing clients (once you have had your initial evaluation) and is HIPAA compliant. This means that your privacy and the security of your personal information is protected.
Business Hours
Monday, Wednesday at office from 9:00am - 12:00pm, at the hospital from 1:00pm - 6:00pm
Tuesday, Thursday, Friday at office from 9:00am - 6:00pm
Occasionally, appointments will be available in the evening or on Saturday. Check the Online scheduling calendar or call the office for possible openings.
Service Fees
Psychotherapy:
Initial Evaluation (90 minutes) - $100.00
Subsequent Therapy Sessions (50 minute) - $85.00 per clinical hour
Students - $55.00 per clinical hour
College Student must present picture I.D.
Adolescent Students Age15 – 17,
Must show government issued picture I.D.
Couples & Family Therapy (90 minutes) - $120.00 per clinical hour
Group Therapy (90 minute sessions) - $40.00 per session
Solution-Focused Counseling – $85.00 per hr
(Can schedule up to 3 hour sessions)
Court Related Services:
Child & Family Investigations – Retainer - $2000, $100.00 per hour
Family Mediation - $125.00 (90 minute session)
Parent Coordination - $125.00 (90 minute session)
Court Testimony and related expenses - $150.00 per hour
Complementary/Alternative Therapies:
Yoga Therapy - $100 (Initial Assessment – 90 minutes)
$85 (60 minutes)
Miscellaneous Charges:
There may be additional fees for lengthy telephone consultations, preparation for
reports for legal cases, extensive copying of records and charts, Please consult your
therapist with questions.
Sliding Scale
Persons with a limited ability to pay for psychotherapy services may use my negotiated sliding scale. The scale is based on the current Federal Poverty Level chart.
Sliding Scale Discounts are only available for individual psychotherapy sessions.
Requirements for receiving discounts include commitment to treatment and documentation for proof of income (The three most current pay stubs for all income earners in the household).
Financial Policies
I truly appreciate your choosing to come to me for therapy. As part of providing high-quality services, I need to be clear about our financial arrangements.
ACCEPTED FORMS OF PAYMENT:
I accept Cash, Check, Mastercard, Visa and Discover. Sorry, I do not accept Medical Insurance at this time. Checks should be made payable to UniQi. The service charge for returned checks is $25.00.
FAILURE TO PAY:
As a therapist, it is my primary goal to provide confidential, compassionate, and effective care to my clients. I adhere to the highest standards of ethical practice and serve your needs in good faith. In order to continue my services to you and other members of our community, I expect payment for services rendered in a prompt manner. If extenuating circumstances arise, please consult with me regarding an acceptable payment arrangement. Failure to do so may result in the need to curtail further sessions until the financial situation is resolved.
It is my policy to keep fees for services as low as possible. For this reason delinquent accounts represent a serious problem. Accounts more than 30 days in arrears will be assessed a monthly finance charge of 1%, which is 12% interest per year or 12.68% effective interest compounded monthly. Accounts more than 90 days in arrears will be referred to collection.
If it becomes apparent that a client does not intend to satisfy his/her financial responsibility, a collection agency or attorney may be contacted to pursue collection of the account. If a fee is charged to UniQi or Wendy McMahon for collection agency services, it will be charged to the client’s delinquent account.
CANCELLED APPOINTMENTS:
UniQi policy requires 24 hours notice for cancellation of any appointment. You can call the office (970)761-0048 or you can, after hours or on weekends, leave a voice mail; my voice mail system is time-stamped. If cancellation is not received on time, a $50 “reserved time” fee will be added to your account, payment of which is due on or before your next visit. Additionally, if you fail to show for a scheduled appointment, the same charge applies.
As a group therapy patient, you are expected to attend weekly. Should circumstances require you to miss group, you will need to call and notify the group leader, otherwise you will be charged for the group.
